Becoming a client... Prospective Clients: Thank you for your interest in Eden-Whild. This is a consulting practice in natural health and herbology. So many people today are turning to natural health for answers. Many of you visit the natural food stores and load up on supplements and herbs. I believe if you self treat with herbs and supplements you not only could waste your money by taking items you dont need, but you run the risk of causing harm and even death. Before you start any program you need to have a history taken and analyzed. You have to know if there is any potential drug and herb interactions. This is what I do! If you want to become a client of Eden-Whild I will send or e-mail a history form. You may also download a copy in adobe Acrobat PDF format here. Please fill it out as best as you can. Every question has a definite purpose. Without a history taken on you it is impossible to help and guide you towards achieving good natural health habits. Click here to print a blank history form out from the internet! The history is made up for either sex, obviously some questions will not pertain to you, so you can skip those. If there are any questions that make you feel uncomfortable, skip those also. But bear in mind this history is kept in your file folder and considered private. Once you establish yourself and have your own chart here, each contact you have with Eden-Whild will be documented. Please send your history back with a check for $100.00, it will take me one to two weeks to work up your personal program. Once I have this done I will send it to you along with a list of supplements and herbs recommended. I will also send along a price list and order form. Be sure you write down what you are taking for supplements, herbs, and medications. Also send along, with your history a 3-5 day diary of everything that you did, ate or drank. |

Return Visit Instructions Returning Clients:It has been brought to my attention that taking time to "come in" for your regular 3-6 month visit is time consuming and inconvenient, especially if you have job and family responsibilities. Also several of my clients move south in the winter and they cannot make their visits. It is important for your health and your program that we communicate on an ongoing basis and make changes as needed. The ultimate goal is to keep you healthy and happy! So, I came up with a plan to keep you all updated and on the right path to good health! Basically what I have started to do is assess or reassess clients over the web, through fax messages, e-mail, snail mail or phone. I am working on a plan to contact you at least every six months and remind you it is time for updating. The charge for these updates and reviews is $50.00 that not only entitles you to your 6 month review's but it entitles you to contact me at any time with concerns or questions and a need for research. I plan to close my office the end of the year and work out of my home, working towards a "correspondence-based" client relationship. Special arrangements can be made for a face-to-face visit if absolutely necessary; I will come to your home, you could come to my home or I could make arrangements to use a friend's office in my present building. There are changes in the natural health field and it is important that you know about these changes. Once I have my web site updated I plan to have newsletters again, however they will be on the web site. For those who do not have access to the web site I would be happy to use snail mail or the telephone. For your routine updates all you need to do is:
